Posterior Reversible Encephalopathy Syndrome in Minimal Change Disease without Hypertension

Abstract
A 64-year-old woman was admitted to our hospital with severe edema, weight gain and frequent voiding. She had severe uremia without hypertension due to unknown causes. On admission day 4, she complained of moderate to severe headache and visual disturbance. Brain magnetic resonance imaging showed reversible vasogenic edema in the occipital areas. All of her neurological symtoms and uremia gradually improved with conservative management. On admission day 20, renal biopsy revealed 6 ? 9 glomeruli, and findings were consistent with minimal change disease. The follow-up brain magnetic resonance imaging was acquired 2 months after discharge, and showed completely resolved radiologic findings. When a patient with severe uremia presents with neurological symptoms, posterior reversible encephalopathy syndrome should be considered in the differential diagnosis even if hypertension is not observed.
